"Aah!" Like Grey's, Howard's injury isn't a fatal wound, but the spear head does slip through and stick into Howard's thigh. He goes down, landing on his knees again, giving Grey the precious seconds necessary to reach him as he struggles to get back up against the pain in his leg and the screaming of his lungs. He's panting too hard to even beg for mercy.
