Kili watches the doctor with a measure of curiosity. His older brother and uncle had had to consider what would happen in the case of death; if Thorin died, that would make Fili the heir apparent, the king, more than just his brother. There had been times when they were younger (and Kili somehow more foolish) when he could simply deny that his brother had any other responsibilities than being his brother, as if that would stem the inevitable. If he simply didn't want Fili to be king, then he wouldn't have to be.
He's still foolish enough to consider briefly that if he simply ignored the thought of dying or death, he could push it off. It only takes a moment, though, before he realizes just how childish it is to think such a thing and how much he misses his brother right about now.
